Trauma and alcohol and other drug use: Why your response matters

Trauma is common in alcohol and other drug treatment, with about 80% of clients seeking help for alcohol and other drug issues having experienced at least one traumatic event and nearly half (45%) screening positive for PTSD symptoms.

Certain groups, including people experiencing homelessness, face even higher trauma exposure levels.

While PTSD often captures our attention, it is just one of many possible mental health responses to trauma exposure; trauma is a deeply personal experience that affects each individual uniquely.

Anxiety and alcohol and other drug use frequently co-occur with PTSD, and without appropriate intervention, people are more likely to develop multiple mental health conditions over time, such as depression, anxiety, and alcohol and other drug use disorders.

And beyond specific mental health conditions, trauma can lead to further issues like anger, grief, social isolation, and difficulties maintaining employment.

Understanding trauma is about recognising the broader psychosocial effects that influence recovery. An effective trauma informed approach in the alcohol and other drug sector is essential.

Trauma informed care incorporates several critical principles:

  • Trauma awareness: Services and workers deeply understand trauma and its widespread impacts and incorporate this understanding into all aspects of their work
  • Safety: Physical and emotional safety are prioritised through privacy, sensitive disclosure, clear roles and boundaries, collaborative approaches, and culturally aware practices
  • Rebuilding control: Service users are provided with opportunities to have choice, be empowered, and foster self efficacy
  • Strengths based: Strengths, coping skills, and strategies that enhance resilience are emphasised

At its core, effective trauma work ensures alcohol and other drug workers understand trauma’s prevalence and impacts, recognise and manage barriers to treatment, and create safe spaces for clients. It’s about stabilising immediate trauma symptoms and aligning both short and long term recovery goals with the right treatment pathways.

Being truly trauma responsive goes beyond simply recognising trauma. It means actively integrating core trauma interventions into everyday care. And no, this doesn’t mean providing specialised trauma therapy in the alcohol and other drug setting. Often, it’s about sensitive screening and assessment, thoughtful psychoeducation, and targeted stabilisation strategies focused on managing changes in beliefs, emotional regulation, and behaviours.
