Brisbane South PHN connected care

In 2022, Brisbane South PHN initiated the Connecting Mental Health and Alcohol and Other Drug project. Brisbane South PHN engaged 360Edge to conduct an environmental scan and literature review to inform this project.

We carried out an environmental scan and literature review to inform the consultation and codesign phases of the Connecting Mental Health and Alcohol and Other Drug project. The project identified opportunities to increase sector cohesion and improve connected care readiness and capabilities within the Brisbane South PHN region.

We have recently been commissioned by Brisbane South PHN to build on this previous work with the development of a connected care service framework and to pilot test and evaluate the operationalisation of the framework. The framework will prioritise addressing access barriers, reducing the impact of stigma, improving service delivery alignment with best practice evidence and integrating the role of lived experience in advisory roles and service delivery capabilities.

The service framework will be designed through a collaborative design approach. It is expected that the pilot will include a skills audit which will inform a workforce development strategy with the aim of building workforce connected care best practice capabilities.
